Job summary

KidsCare Home Health seeks a Pediatric Physical Therapist to provide in-home therapy for children with special needs. You will evaluate patients, develop treatment plans, and educate families to implement home programs, ensuring quality care in a home setting.

Qualifications

• Master’s Degree in Physical Therapy required.
• Current state-issued professional license.
• Valid state driver license.
• CPR certified.
• Pediatric clinical experience preferred.
• Home health experience preferred.

Responsibilities

• Evaluate, organize, and conduct medically prescribed physical therapy for pediatric patients in the home health setting.
• Direct the patient and caregiver in selected tasks to restore, reinforce and/or enhance performance.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress, home programs and discharge planning.
• Refer patients and family members to appropriate services including other professionals, agencies, and resources.
