Job summary

KidsCare Home Health is seeking a Physical Therapist to evaluate, organize, and conduct pediatric home-based therapy for children with special needs. You will direct patients and caregivers in home exercise plans to restore and enhance function.

Qualifications

• Master's Degree in Physical Therapy is required.
• Must hold a current state license to practice physical therapy.
• CPR certification is required.
• Driver's license to travel to patient homes.
• Experience in pediatric physical therapy and home health setting.

Responsibilities

• Evaluate, organize, and conduct pediatric home-based physical therapy sessions.
• Direct patients and caregivers in home exercise programs and discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain progress notes and required documentation for each patient.
• Participate in Quality Assurance and Performance Improvement (QAPI) activities.
• Maintain appropriate caseload and productivity while delivering high-quality care.
