Early Intervention Physical Therapist

Responsible for coordinating treatment and implementing services for assigned clients, including outreach visits, as part of a multidisciplinary Early Childhood Services team.

Primary Functions:

1. Provide outreach services to children and families
2. Consult with team members to develop individualized plans and multidisciplinary services for clients
3. Maintain accurate client records, including IFSPs with assessments, goals, progress documentation, and records of contact with families and collaterals
4. Complete billing documentation as scheduled
5. Adhere to professional standards and guidelines
6. Keep supervisor informed of client progress
7. Meet service requirements based on employment status
8. Follow clinical recommendations and maintain confidentiality
9. Ensure billing and documentation compliance
10. Conduct visits in natural environments based on client and family needs

Qualifications:

• Current licensure as a Physical Therapist by the Massachusetts Board of Allied Health Professions
