Early Intervention Physical Therapist

Responsible for coordinating treatment and implementing services for assigned clients, including outreach visits, as a member of a multidisciplinary Early Childhood Services team.

Primary Functions:

• Provide outreach services to children and families
• Consult with team members to develop individualized plans and multidisciplinary services for clients
• Maintain complete and accurate client records, including IFSPs with assessments and goals
• Document client progress, contact with families and collaterals, and arena assessments
• Complete billing documentation as scheduled
• Adhere to professional standards and guidelines
• Keep supervisor informed of client progress
• Meet service requirements based on employment status
• Follow clinical recommendations and maintain confidentiality
• Ensure billing and documentation compliance
• Conduct visits in natural environments based on client and family needs

Qualifications:

• Current licensure as a Physical Therapist by the Massachusetts Board of Allied Health Professions
