Job summary

THERAPY 2000 is looking for a dedicated Pediatric Physical Therapist to join our mission-driven team delivering home-based therapy to Texas families, focusing on compassionate, evidence-based care.

You will perform comprehensive evaluations, design and implement personalized treatment plans, coordinate with caregivers and other clinicians, and help children reach their fullest potential in the comfort of their home.

Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in a home-health setting.
• Specialty Certifications (example: PCS).
• Bilingual (English/Spanish).

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients’ needs.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs with caregivers.
• Collaborate with patients’ medical neighborhoods and coordinate care with other providers.
• Meet weekly patient visit quotas and complete chart updates and documentation.
• Adhere to agency policies and professional ethics and regulatory requirements.

Responsibilities

What you’ll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
