Job summary

Therapy 2000 is seeking a licensed Pediatric Physical Therapist to provide home-based services across Texas. You will evaluate patients, develop and implement treatment plans, and create effective home exercise programs with caregivers, while collaborating with a multidisciplinary team.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patient needs.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs with patients' caregivers.
• Coordinate care with other providers and disciplines within the patient’s medical neighborhood.
• Meet weekly minimum patient visits as required by the agency.
• Complete chart updates, progress summaries, call logs, and other paperwork on time.
• Comply with agency policies, procedures, and applicable laws for home‑health pediatric PT.

Qualifications

What you’ll need:

• A current/active license to practice Physical Therapy in the state of Texas.
• An innovative spirit complemented by a demonstrated history of reliability.
• Flexibility to shift resources and priorities as necessary, based on the needs of the business.
• Ability to use common workplace technology (laptop computer, phone, email, web browser).

Bonus points for:

• 3+ years as a pediatric Physical Therapist
• Experience in a home-health setting
• Specialty Certifications (example: PCS)

Responsibilities

What you’ll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
