Advocate Health is seeking a Physical Therapist to join the Cleveland Outpatient Rehabilitation team. The full-time role offers 36 hours per week, Monday through Friday, with a pay range of $35.50 to $53.25 per hour.

Requirements include a Bachelor's degree in Physical Therapy, a current state license, and BLS. The therapist will evaluate patients, create care plans, document progress, coordinate with physicians, and supervise PTA's and Rehab Tech/Aide.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Applicable state license required.
• BLS required per policy guidelines.

Responsabilités

• Evaluates patients and develops individualized care plans.
• Gathers and interprets data from medical records and reports.
• Documents evaluations and treatment progress and communicates with physicians.
• Supervises PTA's and Rehab Tech/Aide.
