Advocate Health’s department at 37503 Wilkes Medical Center - Outpatient Rehabilitation seeks a full-time Physical Therapist. The role includes outpatient adult care with weekend rotation to inpatient; opportunities for pediatrics/lymphedema may exist. Requires a PT degree, state licensure, and CPR/BLS.

Provide patient assessments, treatment plans, and documentation per standards while maintaining productivity and quality of care in a supportive hospital environment.

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of applicable state
• CPR certification in Basic Life Support, Level C.

Responsabilités

• Adheres to the general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.
• Provides patient care assessment and treatment with sound professional judgment and realistic goals.
• Maintains accurate documentation including evaluations, progress notes, discharge notes and ICD-10 codes.
• Meets department financial and productivity standards and supports budgeted expectations.
• Participates in process management to improve quality and efficiency of services.
• Promotes professional development and education of other health care workers and shares clinical expertise.
• Communicates effectively with patients, physicians, staff and external customers.
