Job summary

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Evaluates patients and establishes a holistic, evidence-based care plan.
• Gathers and interprets data from medical records, physician reports, patient and family.
• Establishes functional goals with the patient and provides ongoing assessment to adjust the plan of care.
• Documents evaluations, tests, progress notes, education, outcomes, discharge notes, and charges as needed.
• Communicates patient’s evaluation and progress to physicians and other clinical staff; coordinates schedules and discharge planning.
• Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
