Job summary

Community Care, Inc. Home Health is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one pediatric private duty care in a home setting in Pittsburgh, PA. The part-time Day Shift offers stable hours and meaningful patient interactions with a young child.

As an LPN, you will support ADLs, monitor vitals, manage g-tube care and medications, and collaborate with families and the care team to deliver safe, family-centered care. CPR certification and PA license required.

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification.
• Reliable transportation and a valid driver’s license.
• A caring heart, strong independence, and pediatric care focus.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform g-tube care/feedings & aspiration precautions.
• Monitor vitals.
• Monitor seizures.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered pediatric care.
