Qualifications

• Active Colorado Physical Therapist license is required.
• Degree from an accredited physical therapy program.
• BLS CPR certification is required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled physical therapy for children, birth to 21, in home and community settings.
• Conduct evaluations and create individualized treatment plans.
• Educate and support families, empowering them to participate in care.
• Collaborate with a multidisciplinary team to ensure high-quality care.
• Maintain clear and timely documentation.
