Job summary

Chapters Health System seeks a Per Diem RN in the Washington, DC area to provide compassionate hospice care to patients and families. You will assess needs using the nursing process, coordinate with the IDG, and deliver safe, high-quality palliative care across settings, including home visits, hospitals, and hospice facilities.

Qualifications

• Active RN license in the state of assignment.
• Current BLS certification from AHA or Red Cross.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Ability to communicate in English (verbal and written).
• Flexible schedule for Hospice House patients.
• Ability to meet physical requirements and handle lifting/pushing.

Responsibilities

• Provides care to patients and families as part of IDG.
• Coordinates Plan of Care reflecting evolving needs.
• Educates on disease, palliative care, safety and caregiving.
• Reports changes to IDG; initiates physician orders.
• Documents per policy; maintains timely communication.
• Facilitates safe discharge and referrals across services.
