Job summary

Anchor Health in Walnut Creek, CA is seeking an on-call Registered Nurse for the Pediatric Hospice Program in the East Bay. The RN will plan, organize and direct home-based hospice care, coordinating with the interdisciplinary team to implement patient care plans.

The ideal candidate holds a NLN-accredited nursing program degree, CA RN license, CPR, and 1+ year recent nursing experience; hospice or home health preferred. Strong communication and physical endurance are essential.

Qualifications

• Graduate of a NLN-accredited nursing school.
• Current CA RN licensure and CPR certification.
• Minimum 1 year nursing experience within the last 3 years; or have a BSN and a current RN license.
• Minimum of 2 years’ experience, at least one in public health, home care, or hospice nursing is preferred.
• Management experience not required; may supervise hospice aides.
• Excellent observation, verbal and written communication skills, problem solving abilities, and math skills.
• Must be a licensed driver with insured vehicle in good working order.

Responsibilities

• Provide professional nursing care to patients using the nursing process.
• Assess and evaluate patient’s status; initiate and revise the care plan as needed.
• Develop a care plan with goals based on nursing diagnosis and family involvement.
• Act as Case Manager when assigned and coordinate patient care.
• Maintain accurate clinical notes and report changes to the physician.
• Communicate with the physician and community health professionals to coordinate care.
