Job summary

Anchor Health is seeking an on-call Registered Nurse for our Pediatric Hospice Program serving the East Bay. The RN will plan, organize, and direct home-based hospice care, coordinating closely with families and the interdisciplinary team.

Responsibilities include assessing patient needs, updating care plans, and communicating with physicians. CA RN license, CPR, and strong home-health experience are required.

Qualifications

• Graduate of a NLN-accredited nursing program.
• Current CA RN license and CPR certification.
• At least one year of professional nursing experience (last 3 years); or BSN with active RN license.
• Two years in public health, home care, or hospice preferred.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Plan, organize and direct hospice care in the home setting.
• Communicate patient needs to Interdisciplinary team and physicians.
• Coordinate care with family and community providers.
• Supervise hospice aides as needed.
