Job summary

Anchor Health is seeking a Pediatric Hospice RN Case Manager to join our pediatric team. The role focuses on professional nursing care for assigned patients, planning and coordinating care with the IDT and families. Travel to patient visits is required, with occasional educational travel.

Qualifications include an Associates degree (Bachelor’s preferred), CA RN license, CPR/BLS, and pediatric hospice experience. Valid CA driver’s license and TB test results are required.

Qualifications

• Associates Degree required; Bachelor’s degree preferred.
• One year of acute or hospice pediatric experience.
• Current CA RN license.
• Current CA driver’s license with proof of insurance.
• Current CPR/BLS.
• Pediatric experience preferred.
• TB test result within a prior year.

Responsibilities

• Develops written plan of care in coordination with an interdisciplinary Team (IDT).
• Oversees implementation of plan and adjusts as needed to meet patient/family needs.
• Directs care provided by LVN and HA per plan and TEH policy.
• Supervises LVN and HA per policy and regulatory requirements.
• Communicates all changes to IDT and primary MD.
• Maintains timely patient documentation.
• Implements and maintains hospice policies and standards of care.
• Provides health care instruction to patient and family.
• Orients and trains assigned personnel.
• Provides 24/7 coverage to patients/families (on-call).
• Performs admission assessments.
