Job summary

Anchor Health is seeking a Hospice Pediatric RN Case Manager to provide professional nursing care within a pediatric team. You will assess, plan, implement, and evaluate care with input from the IDT, patient, and family, ensuring coordinated communication and care plans with all stakeholders.

Qualified candidates must have CA RN licensure, CPR/BLS, and pediatric hospice experience; travel and on-call duties are involved.

Qualifications

• Associates degree required; bachelor’s preferred.
• One year of acute or hospice pediatric experience.
• Current CA RN license.
• CA driver’s license with proof of insurance.
• Current CPR/BLS certification.
• Pediatric experience preferred.
• TB test result current.
• Bilingual preferred.

Responsibilities

• Develop written plan of care in coordination with the IDT.
• Oversee implementation and evaluate plan to meet patient/family needs.
• Direct care by LVN and HA per patient’s plan of care.
• Supervise LVN/HA per TEH policy and regulatory requirements.
• Communicate changes to IDT and primary MD.
• Maintain timely patient documentation per policy.
• Implement hospice policies and regulatory requirements.
• Provide health care instruction to patient and family.
• Orient and train assigned personnel.
• Provide 24/7 on-call coverage for patients/families.
• Perform admission assessment when necessary.
