Job summary

Anchor Health in California seeks a Pediatric Hospice Team Nurse to support after-hours and some weekend patient care in the home setting. You will plan, organize, and direct hospice services tailored to each patient and family, providing instruction and evaluation of care.

The role requires CA RN/LVN licensure with CPR, clinical notes, and coordination with physicians and community health partners. Strong communication and physical ability to lift and transfer patients are essential; valid

Qualifications

Graduate of NLN-accredited nursing program.
Current CA licensure and CPR certification.
RN/LVN with at least one year of nursing experience in the last three years OR BSN with RN license.
Two years' experience in public health, home care, or pediatric hospice preferred.
Strong observation and communication skills; basic math; nursing skills per competency checklist.
Ability to lift and transfer patients; stand for long periods.
Must be a licensed driver with insured vehicle.

Responsibilities

Provide professional nursing care to patients using the nursing process.
Assess and evaluate patient status.
Complete and submit accurate clinical notes; record pain/symptom changes.
Communicate with physician regarding patient needs and coordinate with community health.
