Anchor Health is seeking a Registered Nurse Case Manager for pediatric hospice in the East Bay. You will plan, organize and direct home-based hospice care, coordinating with the interdisciplinary team and families to implement the patient care plan.

The role emphasizes patient advocacy, communication with physicians, and driving quality care. CA RN licensure, CPR, and nursing experience are required, with BSN as an alternative.

Qualifications

• Graduate of a National League of Nursing accredited school of nursing.
• Current licensure in CA and CPR certification.
• RN with 1+ year in last 3 years or a BSN with RN license.
• 2+ years preferred in public health, home care, or hospice nursing.
• No management experience required; may supervise aides.
• Must be a licensed driver with reliable vehicle.

Responsibilities

• Provide professional nursing care to patients using the nursing process.
• Assess and evaluate patient status and plan care accordingly.
• Initiate and revise the plan of care as patient needs change.
• Develop care plans with goals involving patient and family.
• Coordinate patient care as Case Manager for assigned caseload.
• Complete accurate clinical notes and document changes.
• Communicate with physicians and community health personnel to coordinate care.
