Job summary

Anchor Health is seeking a Pediatric Hospice RN Case Manager to provide professional nursing care to assigned pediatric patients and coordinate care with the interdisciplinary team. You will assess, implement, and evaluate plans of care and communicate changes to the IDT and primary MD.

Travel is required for patient visits. CA RN license, CPR/BLS, and a valid CA driver’s license with insurance are required; bachelor’s degree preferred and pediatric hospice experience is valued.

Qualifications

• Associates degree required; Bachelor’s preferred.
• At least 1 year of acute or hospice pediatric experience.
• Current CA RN license and CA driver's license with insurance.
• Current CPR/BLS certification.
• Pediatric experience preferred; TB test result required.
• Bilingual preferred.

Responsibilities

• Develops written plan of care with the IDT.
• Overs the implementation and evaluates the plan to meet patient/family needs.
• Directs care provided by LVN and HA per plan.
• Supervises services per TEH policy and regulatory requirements.
• Communicates changes to IDT and primary MD.
• Performs duties per competency checklist and standards of care.
• Maintains timely patient documentation using point‑of‑care charting.
• Implements and maintains hospice policies and regulatory requirements.
• Provides health care instruction to patient and family.
• Orients and trains assigned personnel.
• Participates in 24/7 coverage; on-call and census fluctuation.
• Performs admission assessment when necessary.
