Job summary

Anchor Health is seeking a Pediatric Hospice RN Case Manager to join our clinical team in California. You will assess patients with input from the IDT, develop and adjust care plans, coordinate services, and ensure effective communication with families and providers.

Travel required for patient visits, with on-call coverage as needed. Ideal candidates hold an Associate degree (Bachelor preferred), CA RN license, CPR/BLS, and hospice pediatric experience; bilingual skills are a plus.

Qualifications

• Must have Associates Degree; Bachelor Degree preferred.
• Must have one year of acute or hospice pediatric experience.
• At least 12 months experience with 1-2 years in acute care.
• Current CA RN license.
• Current CA driver’s license with proof of insurance.
• Current CPR/BLS certification.
• Prefer certification as hospice nurse.
• Pediatric experience preferred.
• Proof of recent TB test result.
• Computer skills (word processing), calculator, adding machine, fax, photocopier.

Responsibilities

• Develops written plan of care in coordination with an interdisciplinary team (IDT).
• Oversees implementation of plan, evaluates and adjusts as needed.
• Directs care provided by LVN and HA per plan.
• Supervises LVN and HA per TEH policy and regulatory requirements.
• Communicates all changes to IDT and primary MD.
• Maintains timely patient documentation with electronic charting.
• Implements hospice policies and regulatory requirements.
• Provides health care instruction to patient and family.
• Trains assigned personnel and participates in on-call coverage.
• Performs admission assessment as needed.
