Job summary

Anchor Health in Concord, CA is seeking a Pediatric Hospice RN Case Manager to provide comprehensive nursing care to pediatric patients within an interdisciplinary team. You will assess, implement, and evaluate care plans, ensuring effective communication with families and the care team to meet patient needs.

This role requires CA RN license, pediatric hospice experience preferred, and willingness to travel for patient visits.

Qualifications

  • Must have an Associates Degree; Bachelor’s is preferred.
  • Must have one year of acute or hospice pediatric experience.
  • Current CA RN license and CA driver’s license with insurance.
  • Current CPR/BLS certification; hospice nursing certification preferred.
  • Pediatric experience preferred; TB test result recent.

Responsibilities

  • Develops and coordinates the plan of care with the IDT.
  • Oversees plan implementation and evaluates patient/family needs.
  • Communicates changes to IDT and primary MD.
  • Maintains patient documentation with electronic charting.
  • Implements hospice policies and standards of care.
  • Provides care instructions to patients and families.
  • Orients and trains assigned personnel; conducts admissions.
  • Travels to patient visits as required.

PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION
Overview

The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. Ensures appropriate communication and plan of care coordination to all internal/external customers

JOB DUTIES/KNOWLEDGE/JOB PERFORMANCE:
  • Develops written plan of care in coordination with an interdisciplinary Team (IDT).
  • Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs.
  • Communicates all changes to IDT and primary MD.
  • Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice.
  • Maintains timely patient documentation using "point of care" electronic charting.
  • Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.
  • Provides health care instruction to patient and family members.
  • Orients, instructs, and trains assigned personnel.
  • Performs admission assessment and admission when necessary.
DEGREE OF TRAVEL:
  • Travel required for patient visits.
  • Occasional travel for educational purposes.
QUALIFICATIONS:
  • Must have Associates Degree, Bachelor Degree preferred.
  • Must have one year of acute or hospice pediatric experience
  • At least 12 months experience with 1-2 years preferred in acute care.
  • Current CA RN license.
  • Current CA driver's license with proof of insurance.
  • Current CPR/BLS
  • Prefer certification as hospice nurse.
  • Pediatric experience preferred.
  • Proof of recent TB test result
  • Bilingual preferred.
