Job summary

Compassus is seeking a Registered Nurse for hospice care in California, serving patients in home settings across designated coverage areas. You will lead assessments, plan and direct patient care in collaboration with the IDG, and ensure high-quality end-of-life care for diverse patient groups.

Ideal candidates have CA RN licensure, at least 1 year in hospice or related fields, and strong communication skills to coordinate with families and care teams.

Qualifications

• CA RN license required; BLS within 30 days of hire.
• 1 year of hospice, palliative care, home health, or skilled nursing experience preferred.
• Strong communication skills and ability to work with diverse teams.

Responsibilities

• Completes comprehensive patient and family assessments to determine hospice needs.
• Develops, initiates, and revises Plans of Care with IDG collaboration.
• Administers medications and treatments and counsels patients/families on care needs.
• Documents patient condition, care provided, and outcomes timely.
• Coordinates with IDG and Patient Care Manager to ensure continuity of care.
• Participates in on-call duties and maintains productivity per guidelines.
• Mentors and supports team members and contributes to performance improvement.
