Job summary

Children’s Health seeks a Health Services Coordinator to enhance patient continuity of care. This role collaborates with the care team to ensure continuous, comprehensive, and coordinated services across ambulatory care management.

You will support referrals, transitions, follow-ups, and effective communication with providers and families. The position requires a minimum of two years in healthcare and a high school diploma, with preferred pediatric/Family practice background and a Medical

Qualifications

• At least 2 years experience in a healthcare setting required.
• At least 4 years experience at a Pediatric/Family Practice or on a Pediatric Med/Surg floor preferred.
• High School education required

Responsibilities

• Outreach to patients to recommend needed services
• Act as liaison between health services and community to facilitate patient access
• Facilitate communication and patient/caregiver empowerment in interactions with healthcare systems
• Assist patients to improve health outcomes and reduce unnecessary ED utilization
• Coordinate and communicate with Care Team on care plan activities, referrals, transition planning, and follow-up tracking
• Review Disease Management Registries to identify patients with gaps in care across clinics
• Notify RN Care Managers, PCPs and staff of issues pertinent to patient care
• Work with clinics to determine outstanding needs and integrate activities with clinic Operations
• Follow HIPAA rules when providing PHI; verify numbers and proper identification
• Document conversations and denials in tools, inform team of denials and actions
• Other tasks as assigned
