Job summary

Rutgers University in Camden South-Cherry Hill, NJ, seeks a Health Care Case Manager (Registered Nurse) to support the Child Health Program within the School of Nursing. The role collaborates with DCF/CP&P to meet the health care and protection needs of children in out-of-home placement.

Responsibilities include enrolling children with managed care, assessing health status, coordinating medical home and EPSDT visits, and linking families to health resources. NJ RN licensure and BLS are required.

Qualifications

• Current licensure as a Registered Nurse in New Jersey is required.
• Bachelor’s Degree in Nursing or health-related field required; pediatric experience may substitute.
• Minimum 1–2 years of full-time clinical pediatric nursing experience.
• Current American Heart Association BLS certification required.
• Vehicle and valid New Jersey license required for off-site visits.

Responsibilities

• Assist enrollment in managed care and identify a medical home for each child.
• Assess health status by reviewing records and interviewing providers and caregivers.
• Schedule Initial Physical Health Evaluations with care team and DCF/CP&P involvement.
• Develop a child health care plan with medical home and specialists.
• Link child and caregiver to appropriate health resources and services.
• Ensure EPSDT visits and specialty appointments occur and reports are forwarded.
