Job summary

Middlesex County is seeking a dedicated Case Manager to join our Special Child Health Services (SCHS) team in New Jersey. The role coordinates services to promote health, development, independence, and well-being for Children and Youth with Special Health Care Needs (CYSHCN) and their families.

The ideal candidate is a licensed RN or CSW with pediatric case-management experience and a family-centered approach. Bilingual Spanish/English proficiency is preferred.

Qualifications

• RN license in New Jersey or CSW/LSW/LCSW license required.
• Bachelor's or Master's degree in Nursing or Social Work.
• Minimum of one year of pediatric case management experience.
• Experience coordinating across medical, developmental, educational, behavioral health, and/or social service systems.
• Bilingual Spanish/English proficiency preferred.

Responsibilities

• Conduct needs assessments across developmental, educational, medical, behavioral, and emotional domains.
• Develop, implement, and update Individual Service Plans (ISPs) to reflect family goals.
• Coordinate services with medical, developmental, educational, behavioral health, and social service providers.
• Advocate for timely access to services and supports for children and families.
• Follow up on referrals and coordinate care among providers and agencies.
• Provide crisis support and assist families in identifying resources and solutions.
• Facilitate communication within the care team while maintaining confidentiality and professionalism.
• Monitor progress and adjust services as needs evolve.
• Engage with community partners to strengthen support networks for CYSHCN.
