Qualifikationen

• Current licensure as a Registered Nurse in New Jersey.
• BSN or health-related bachelor's degree; pediatric nursing experience valued.
• 1–2 years of full-time pediatric nursing experience preferred.
• American Heart Association Basic Life Support (BLS) certification.

Aufgaben

• Assist with enrollment of a child in a managed care entity and identify a medical home.
• Assess health status by reviewing records and interviewing caregivers; share results with care team.
• Schedule an Initial Physical Health Evaluation with caregiver and CP&P.
• Develop a child health care plan with medical home and specialists.
• Link child and caregiver to health resources and community services.
• Ensure timely EPSDT visits and specialty appointments; forward reports as needed.
