Job summary

PM Pediatric Care in New York seeks a Clinical Therapist (LMSW, LCSW, LMHC) to join our Behavioral Health Program. The role focuses on providing comprehensive behavioral health for common primary care conditions within an extended primary care collaborative model.

Responsibilities include psychosocial assessments, counseling, and documentation, with a caseload of 30-35 weekly patients and 5 daily at minimum.

Qualifications

• Master's Degree in Social Work or a related field.
• Relevant licensure in NY: LMSW, LCSW, or LMHC.
• Experience with pediatric and adolescent patients.
• Experience providing teletherapy to children and adolescents preferred.

Responsibilities

• Perform psychosocial assessments based on evidence-based practices.
• Provide counseling and therapy using approved modalities.
• Work as individual, family, and group therapist.
• Complete required documentation including biopsychosocial assessments and treatment plans.
• Maintain a caseload of 30-35 patients weekly and a minimum of 5 daily.
• Collaborate with patient, family, and care team to overcome obstacles in treatment.
• Maintain compliance with company policies and procedures.
