Job summary
Children's Health in Dallas seeks a Clinical Pharmacist to oversee pharmaceutical services in assigned areas. You will join a multidisciplinary team to promote safe, rational, and effective medication use and support patient care across inpatient and outpatient settings.
Key duties include reviewing orders, optimizing therapy, delivering drug information, and participating in quality improvement initiatives.
Qualifications
- Bachelor's degree or Pharm.D. from an accredited school of pharmacy (B.S. or Pharm.D).
- Pharmacy Doctorate with pediatric residency preferred.
- ACPE IV training within 90 days (except Retail).
Responsibilities
- Review orders for appropriateness and final verification.
- Assess effectiveness, safety and affordability; optimize therapy with care team.
- Provide drug information to physicians, nurses, and patients/families.
- Monitor for drug interactions and duplications in inpatient/outpatient settings.
- Participate in transitions of care and medication access issues.
- Support education to staff and precept learners; contribute to quality initiatives.
