Children's Health in Dallas, TX, is seeking a Clinical Pharmacist for the Central Inpatient Pharmacy. This on-site role supports medication therapy optimization, patient counseling, and collaboration with the healthcare team.

Responsibilities include order verification, drug information delivery, and participation in transitions of care. Requires a Pharm.D or equivalent and Texas license; pediatric residency preferred.

Qualifications

• Four-year degree in pharmacy or Pharm.D from an accredited school.
• Texas Pharmacist license required.
• ACPE IV training within 90 days.
• Hospital pharmacy experience preferred.

Responsibilities

• Review orders for appropriateness and complete final verification.
• Assess effectiveness, safety and affordability; optimize therapy.
• Provide drug information to physicians, nurses and families.
• Monitor inpatient and outpatient profiles for interactions and duplications.
• Participate in transitions of care and ensure access to medications.
• Precept students and deliver education to staff; support research.
