Job summary

Children's Health is seeking a Clinical Pharmacist (HEM-ONC) in Dallas, TX, on-site. The role focuses on safe, rational medication use within a multidisciplinary team, including pediatric hematology/oncology patients, with responsibilities spanning drug info, therapy optimization, and transitions of care.

The position requires hospital pharmacy experience (1 year preferred), a Pharm.D or B.S. in pharmacy, Texas license, and completion of a pediatric rotation; Epic order entry experience is

Qualifications

• Pharmacist licensure in TX or eligibility.
• Experience in hospital settings preferred (1 year).
• ACPE IV training required within 90 days of hire.

Responsibilities

• Review orders for appropriateness and perform final verification.
• Assess effectiveness, safety and affordability of medications; optimize therapy.
• Provide drug information to physicians, nurses and patients; monitor interactions.
• Participate in transitions of care and patient access issues.
• Engage in quality improvement activities and adhere to regulatory requirements.
• Educate staff and precept learners; contribute to literature and research.

Education

Bachelor's Degree (B.S. or Pharm.D) from an accredited school of pharmacy

PhD Degree Doctorate of Pharmacy and completion of a pediatric clinical pharmacy residency

Required Training/Hours Essential 20 hour ACPE accredited IV training course
