Qualifications

• Master's or Bachelor's degree with relevant experience required.
• Valid Driver's License required.
• Eligible for Medicaid Provider Number and enrollment with Children's Medical Services.
• Requires Level II Criminal Background Screening.

Responsibilities

• Conduct screenings and complete family assessments within a multidisciplinary team.
• Update IFSPs, coordinate services, and access community resources.
• Participate in developmental evaluations/screenings and intake planning to determine eligibility.
• Work with IFSP team to meet State/Federal timelines for service delivery and transitions.
• Maintain accurate client records, including program plans, notes, and billing.
