Job summary

University of Florida is seeking a Clinical Case Manager I- Peds Early Steps to lead screening and family assessments, coordinate services, and manage IFSPs for children referred through IDEA Part C. You will work within a multidisciplinary team to determine eligibility and ensure timely service delivery in compliance with AHCA guidelines.

Qualifications

• Master’s degree in an appropriate area; or a Bachelor’s degree in an appropriate area of specialization and two years of relevant experience.
• Valid Driver’s License.
• Be eligible for a Medicaid Provider Number, and be eligible for enrollment with Children’s Medical Services.
• Requires Level II Criminal Background Screening.

Responsibilities

• Conduct screenings, complete family assessments, and provide case management.
• Serve as a multidisciplinary team member to determine eligibility during evaluations.
• Update IFSPs and coordinate services; access community resources for children referred to the Northeastern Early Steps Program through IDEA Part C.
• Maintain accurate client records including program plans, intervention notes, case notes, and billing information.
• Participate in local and statewide meetings and trainings to ensure best practice and compliance.
