Job summary

Children's National Hospital is seeking a Case Manager II in Washington, DC. You will support patients and families throughout their healthcare journey, coordinating care with multidisciplinary teams and focusing on assessments, planning, and follow-up to improve outcomes.

The role emphasizes discharge planning, community linkage, and documentation within a high-volume pediatric setting. A Master’s degree and DC RN licensure are required, with CM/UM certification preferred.

Qualifications

• Master’s Degree required.
• Five years’ experience with at least three years in an acute inpatient setting required.
• RN license in DC required; Case/Utilization Management certification preferred.

Responsibilities

• Provide professional case management services focusing on assessing needs, care planning, and targeted interventions.
• Collaborate with multidisciplinary teams to coordinate services and ensure follow-up for continuity.
• Engage in community outreach to connect families with essential resources and services.
• Develop discharge and transitional care plans to facilitate smooth care transitions.
• Document and participate in performance improvement processes within the Nursing Division.
