Job summary

Full Circle Health in Boise, ID is seeking a Pediatrics Case Coordinator to coordinate comprehensive, family-centered care for children and adolescents with complex health and social needs. You will build individualized care plans and connect patients with healthcare and community resources.

Collaborating with families, providers, schools, and community partners, you will identify barriers to care, manage transitions, and monitor outcomes to improve health and continuity of care for pediatric

Qualifications

• Two years minimum of clinical practice.
• Experience in population health, case management, or transitions of care preferred.
• Pediatrics experience preferred but not required.

Responsibilities

• Coordinate comprehensive, family-centered care for pediatric patients.
• Develop and manage individualized care plans.
• Facilitate access to healthcare and community resources.
• Address barriers to care and support transitions of care.
