Job summary

Bluebird Kids Health in Florida seeks a dynamic Care Coordinator for the CARES Center to support families, coordinate care, and ensure timely follow-ups in a fast-paced, growth-oriented setting.

The ideal candidate has strong interpersonal skills, prior medical receptionist experience, and a proven ability to deliver results while maintaining HIPAA-compliant records and documentation in the EMR.

Qualifications

• Must be a self-starter with strong interpersonal skills.
• Experience in a medical receptionist role.
• Ability to deliver results in a fast-paced, growth-oriented environment.

Responsibilities

• Serve as a key administrative point of contact for care coordination needs.
• Manage the provider inbox, route items, and track to completion.
• Handle inbound/outbound calls and schedule follow-ups with families.
• Track needs for patients requiring frequent follow-up.
• Coordinate outside appointments and diagnostics, gather paperwork, and support prior authorizations.
• Complete referrals and prior authorizations as required by insurance.
• Support families seeking mental/behavioral health services with information and resources.
• Manage prescription refill requests per clinic workflow and document in EMR.
• Maintain patient records and verify demographics during interactions.
• Document interactions clearly in EMR following HIPAA.
