Job summary

AdventHealth in Orlando is seeking an RN Care Coordinator to support Pediatric Specialty Programs and multidisciplinary clinics. The role focuses on planning and coordinating care across the continuum, partnering with families and health professionals for seamless transitions from admission to discharge.

The ideal candidate will hold an RN license (ADN/BSN), have 2+ years of pediatric care coordination experience, and demonstrate strong communication and collaboration skills.

Qualifications

• Excellent verbal, written and interpersonal communication skills.
• Capable of relating to diverse age and social backgrounds.
• Professional knowledge and clinical ability sufficient to provide excellent patient care and family support activities.
• An independent work initiative, sound judgment, diplomacy, and a professional demeanor.
• Sound knowledge of procedures and regulations specific to the assessment and care of infants, children and adolescents.

Responsibilities

• Coordinate pediatric specialty programs and multidisciplinary clinics to ensure seamless patient care.
• Plan and implement care utilizing community resources and coordinate with health professionals.
• Maintain nursing standards and contribute to policy and protocol development and evaluation.
• Contribute to quality improvement initiatives and educational opportunities for staff and community.
