Job summary

Qualifications

• Bachelor Degree in nursing from an accredited University is required.
• Master’s Degree in Nursing is preferred.
• Certification in Case Management is preferred.
• BLS and RN licensure required.

Responsibilities

• Delivery of care coordination services for patients in the clinic setting.
• Plan and deliver family-centered patient care through collaboration with patient, family, nurses, social workers, physicians, other practitioners, caregivers and the community.
• Advocate for optimal health, access to care, and appropriate resources.
