Job summary

CVS Health Corporation seeks a dedicated Case Management Coordinator to support OhioRISE services. This full-time field-based role focuses on care coordination, education and collaboration with families, providers and community partners to improve member outcomes.

Based in central or southeastern Ohio with up to 50% travel to member homes and other sites, the position requires 8am–5pm hours, flexibility for after-hours work, and a strong commitment to quality, safety and respectful, culturally

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with computers and MS Office applications.
• Bachelor's degree or non-licensed master level clinician required.
• Willing to travel within Ohio up to 50% and beyond core hours as needed.

Responsibilities

• Collaborate with care managers to support and educate members.
• Coordinate care plans and monitor progress for complex cases.
• Engage with families, providers, and community resources to improve outcomes.
• Document care activities in alignment with regulatory guidelines.
