Job summary

St. Mary’s Healthcare System for Children is seeking a Patient Service Coordinator for the Centralized Para Placement Unit.

The role ensures initial placement of paraprofessional services from contracted vendors and reassignment as needed, building rapport with vendors to ensure coverage for all cases in a timely, proportional manner. The position involves collaborating with Home Care Teams, managing vendor profiles, and coordinating scheduling in Cerner HomeWorks.

Qualifications

• Knowledge of hardware, software, and telecommunications operations.
• Strong knowledge of Microsoft Office applications.
• Excellent consultative and communication skills.
• Analytical ability, judgment, and discretion with confidential matters.
• Ability to work efficiently with management and staff.

Responsibilities

• Communicates with paraprofessional vendors to cover cases promptly.
• Collaborates with Home Care Team to set service levels and schedules.
• Ensures vendor profiles meet policies and obtains supervisor signatures.
• Informs scheduling system entries and assigns vendors in the scheduling module.
• Monitors service changes and maintains a billing log for each patient.
• Provides back-up support when the team or scheduler is out.
• Keeps senior management updated on cases with coverage gaps.
• Participates in meetings and contributes to process improvements.
• Participates in special projects.
• Performs other related duties as assigned.
