Job summary

CMC Phoenix is seeking a full-time Discharge Planner to join the Case Management team in Phoenix. The role focuses on coordinating care along the continuum, helping patients access appropriate resources and achieve optimal health.

Responsibilities include collaborating with physicians, therapists, and families, arranging admissions and transfers, and maintaining accurate patient records. The position emphasizes compassionate, patient-centered planning and effective communication across care

Qualifications

• Bachelor’s degree in sociology or psychology required.
• Minimum of one year of experience in a behavioral health environment.
• BLS certification required upon hire.
• CPI certification to be obtained within 30 days of hire.

Responsibilities

• Provide clerical support to Case Managers and Social Workers.
• Collaborate with physicians, therapists, treatment team members, and patients to develop effective continuing care plans.
• Assist with telephone calls, faxing, copying, and coordination of discharge plans.
• Present aftercare options to patients and discuss with individuals or families as needed.
• Collect, release, and manage patient information for accurate research and referrals.
• Maintain timely and accurate patient records.
• Initiate contact and coordinate with referral sources, mental health providers, and various care facilities to ensure continuity of care.
• Arrange admissions and transfers to hospitals, treatment centers, and other placements.
• Schedule follow-up appointments for outpatient treatment.
• Attend and contribute to treatment team meetings professionally and punctually.
• Build and maintain positive relationships with community resources and referrals.
• Conduct research and maintain updated provider and resource lists and databases.
