Qualifications

• Minimum of one year experience working with a frail or elderly population.
• High School Diploma or GED; CPR and First Aid certification required.
• Is medically cleared for communicable diseases and up-to-date immunizations.

Responsibilities

• Coordinate training and development for Personal Care Attendants (PCA).
• Assist with PCA orientation and ADL training.
• Maintain plan of care for participants.
• Attend Inter Disciplinary Team (IDT) meetings as PCA proxy.
• Ensure compliance with CMS guidelines and center policies.
