Job summary

AltaMed Health Services Corp. is seeking a Personal Care Attendant II to provide and support personal care ADLs/IADLs for PACE participants, across centers, homes, or virtual visits.

The role requires coordination with the care team to meet care plan goals and improve participants' independence and social engagement. The PCA II will back up PCA I as needed and communicate with IDT members to report health status, supplies, and social needs, ensuring compassionate, coordinated care for all

Qualifications

• Certified Nursing Assistant education and training as required by CA DHHS.
• High school diploma required or some college education/credits preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred depending on location.

Responsibilities

• Perform and assist with personal care ADLs/IADLs for participants in the PACE program across care delivery platforms (center, home, or virtual).
• Assist the PCA I and support care coordination by gathering and reporting information related to care plans, health status and well-being.
• Collaborate with IDT members to address participant needs and foster independence and socialization.
