Job summary

Amedisys is seeking a patient services coordinator to manage scheduling for patient visits, coordinate with clinicians, and support seamless care coordination across care centers. The role emphasizes timely Oasis visits, efficient geography planning, and strong communication with patients and staff.

Qualifications

• High School diploma or GED equivalent.
• Preferred: one year of scheduling experience in a healthcare environment.
• One year of administrative experience in a health care environment.

Responsibilities

• Ensures patient assessment visits including Oasis visits are scheduled and performed timely.
• Ensures clinicians are assigned and scheduled in the most efficient geography, maximizing clinician efficiency and matching skill with required care.
• Works collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center.
• Manages calls from patients and field staff related to scheduling issues.
• Refers clinical and performance related issues to the clinical manager.
• Monitors hospitalized patients, communicating and ensuring the team follows up as necessary.
• Supports clinical manager and ensures timely communication with patients, clinicians, referral sources, and other office personnel.
• Runs all applicable reports and responds to workflow, taking appropriate actions.
• Assists with internal or external transfer of patients between care centers and/or hospice services.
• Maintains supply closet with routine supplies and serves as liaison with the field for patient supply needs.
• Performs other duties as assigned.
