Qualifications

• High School diploma or GED required.
• One year scheduling experience in a healthcare environment.
• One year administrative experience in a health care environment.

Responsibilities

• Ensures patient assessment visits including Oasis visits are scheduled and performed timely.
• Ensures clinicians are assigned and scheduled efficiently by geography and care needs.
• Coordinates with field clinicians to complete visits as ordered and within the center.
• Manages calls from patients and field staff related to scheduling issues.
• Refers clinical and performance related issues to the clinical manager.
• Monitors hospitalized patients and ensures follow-up as necessary.
• Supports communication with patients, clinicians, referrals, and other offices.
• Runs reports and responds to workflow with appropriate actions.
• Assists with transferring patients between care centers or hospice services.
• Maintains supply closet and liaises with field for patient supply needs.
