Qualifications

• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Schedule patients to branch field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care coordination and continuity of care.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
