Amedisys Home Health is seeking a Patient Services & Home Health Scheduler to be the central hub for coordinating patient assessments and clinician schedules. The role ensures timely visits, keeps communication flowing across patients, clinicians, and care teams, and thrives in fast-paced environments with strong problem-solving skills.

In this office-based position, you will support efficient workflows, align schedules by geography, and contribute to reliable patient care.

Qualifications

• High School diploma or GED required.
• Experience in healthcare scheduling or administration preferred.
• Excellent communication and coordination skills.
• Ability to solve problems quickly in a fast-paced environment.

Responsibilities

• Schedules patient assessment visits, Oasis visits, and ensures timely execution.
• Assigns and schedules clinicians by geography to maximize efficiency.
• Collaborates with field clinicians to complete visits as ordered and within care center.
• Handles calls from patients and staff regarding scheduling issues.
• Escalates clinical and performance issues to the clinical manager.
• Monitors hospitalized patients and coordinates follow-ups as needed.
• Maintains timely communication with patients, clinicians, referrals, and teams.
• Generates reports and responds to workflow needs.
• Assists with patient transfers and equipment/supply coordination as required.
• Performs other duties as assigned.
