You will collaborate with field clinicians, manage scheduling communications, and help maintain smooth operations across care centers to support patients at home.

Qualifications

• High School diploma or GED required.

Responsibilities

• Schedules patient assessment visits including Oasis visits.
• Assigns and schedules clinicians to maximize efficiency and match care needs.
• Coordinates with field clinicians to ensure visits are completed as ordered.
• Handles calls from patients and staff regarding scheduling.
• Escalates clinical issues to the clinical manager as needed.
• Monitors hospitalized patients and ensures follow-up communications.
