Responsibilities
- Ensure patient assessment visits, including all Oasis visits, are scheduled and performed timely.
- Assign and schedule clinicians efficiently, maximizing their capacity and matching skills with care needs.
- Collaborate with field clinicians to ensure all patient visits are scheduled and completed as ordered.
- Manage calls related to scheduling issues from patients and staff.
- Refer clinical and performance issues to the clinical manager.
- Monitor hospitalized patients and coordinate follow-up as needed.
- Support clinical communication with patients, clinicians, and referral sources.
- Generate and respond to workflow reports.
- Assist with patient transfers between care centers or hospice services.
- Maintain supplies, ensuring they are within expiration and properly stored, acting as liaison for patient supply needs.
- Perform other duties as assigned.
Qualifications
- High School diploma or GED.
Preferred
- One year of scheduling experience in healthcare.
- One year of administrative experience in healthcare.
