Job summary

UCLA Health is seeking a dedicated liaison to support physicians and patients in assigned primary and specialty care practices. You will help patients navigate care, coordinate scheduling and referrals, and ensure smooth information flow across the health system.

You will facilitate referrals to appropriate resources, provide patient follow-up, and communicate with referring physicians. Proficiency with EPIC/Care Connect is preferred to support efficient care delivery.

Major duties include but are not limited to:

Salary Range: $31.88 - 45.66/hourly
