The Per Diem Patient Service Coordinator will be based out of our office in West Orange, NJ and is responsible for scheduling rehab who will be providing medical services to patients within Bergen County, in accordance with agency procedures and policies.

Responsibilities
  • Responsible for scheduling assignments in an efficient and coordinated manner in compliance with departmental scheduling guidelines and territory assignments.
  • Retrieves voice mail messages including call-offs, visit statistics, changes in staff’s schedules, etc. Processes messages accordingly and reports information to Scheduling and/or Clinical Manager as appropriate.
  • Prepares, retrieves and prints charts as necessary for scheduling assignments.
  • Prepares and maintains scheduling books/scheduling calendars per scheduling guidelines
  • Maintains accurate and complete staffing log with current information in the scheduling database.
  • Verifies accuracy of patient information and patient availability for home visits. Communicates scheduled appointments and staff assignments to patients.
  • Ensures assignments support productivity standards
  • Maintains appropriate communication with Scheduling and/or Clinical Manager regarding scheduling problems and other patient related matters
  • Enters data into EMR to maintain accuracy of clinical information.
  • Maintains confidentiality of patient information.
  • Performs other duties as assigned.
Qualifications

Qualified candidates must:

  • Knowledge of medical terminology required.
  • Proficient with Microsoft applications – excel, outlook and office.
  • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration.
  • Strong planning and organizational skills.
  • Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to maintain strict confidentiality.

Education/Training/Experience:

  • High School Diploma or GED.
  • Prior experience in home care and healthcare staff scheduling preferred
